Please read the statements below. By booking with us you agree to all of the following statements:
- I give my consent for me and/or my child to take part in activities led by Adventure Pirate LTD.
- I/my child will wear all the appropriate personnel protection/safety equipment that Adventure Pirate LTD provide.
- I give my consent to photographs of me/my child being used to promote Adventure Pirate LTD activities.
- I will inform Adventure Pirate leaders/instructors of any medical conditions that may affect my/my child’s participation in Adventure Pirate Activities.
- I consent to me/my child receiving any necessary medical treatment for any injury or illness during the party. You will be contacted immediately if your child is injured or becomes ill.
- I understand that Adventure Pirate activities are physically demanding.
- I understand that there is a risk of accidental injuries/illnesses whilst taking part in Adventure Pirate Activities. These include, but are not limited to breaks, strains, cuts, grazes, bruises and feeling too warm or too cold.
- I understand that a strict code of practise for working with children will be followed and all activities are risk assessed. Risk assessments are available upon request.
- Adventure Pirate instructors will encourage positive behaviour at all times, including team working, tolerance and respect for views and opinions of others.
